Position Summary
Responsible for providing occupational therapy evaluation and treatment to patients in their homes or alternate community settings.
Principal Responsibilities
- Support organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers.
- Assess and evaluate clients for occupational therapy needs and develops and implements appropriate plan of action in collaboration with the physician.
- Document all changes in client’s condition and updates care plans and outcomes accordingly.
- Identify patient, caregiver, and family counseling and education needs and plans appropriate teaching strategies.
- Monitor quality and appropriateness of services provided.
- Participate in VNAHG’s QAPI and in-service programs.
- Adhere to agency infection control and safety policies.
- Adhere to the organization’s policy in regards to absenteeism and appearance.
Job Competencies & Minimum Qualifications
- Occupational Therapy Licensure in NJ
- 2-3 years of prior clinical experience and community based experience preferred
- Strong computer skills
- Strong communication and interpersonal skills
- Valid NJ drivers license

Pay
In compliance with New Jersey's Pay Transparency Act, the compensation range for this position is USD $84,000 to USD $112,000.
