Responsibilities
- Perform comprehensive assessments of residents' functional abilities, focusing on activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
- Develop tailored intervention plans to improve residents' independence in daily activities.
- Ensure accurate and consistent collection of Section GG data for Annual, Quarterly, Falls, Significant Change assessments, BIMS, PHQ 2-9, and other clinical data as needed.
- Administer OT interventions, including ADL training, adaptive equipment provision, and environmental modifications.
- Educate residents, family members, and care staff on safe mobility techniques and exercises for continued progress.
- Conduct remote check-ins through remote therapeutic monitoring (RTM) with discharged patients to home or the next level of care.
- Maintain detailed and up-to-date documentation of resident evaluations, treatment plans, daily and progress notes, and HMO/Managed care progress updates in EMRs (Net Health/PCC).
- Collaborate with other rehab staff, including Physical Therapists and Speech-Language Pathologists, for holistic resident care.
- Comfortable and knowledgeable with Point of Service (PoS) care delivery and documentation.
