About the role

Location: Ventura County (Oxnard, Camarillo, Thousand Oaks, Moorpark). Schedule: Per‑Diem. Remedy Home Health Care is a leading provider of skilled nursing services in Los Angeles County and Orange County, dedicated to delivering high‑quality care in the comfort of patients’ homes. Our mission is to help patients regain independence, improve quality of life, and feel safe at home through personalized, compassionate care. We are seeking compassionate, licensed Occupational Therapists to deliver patient‑centered care directly to clients in their homes.

Responsibilities

• Regain independence through customized therapy plans
• Improve functional abilities for activities of daily living (ADLs)
• Recover from injury, surgery, or hospitalization in the comfort of home
• Reduce fall risk and improve home safety
• Perform in‑home OT evaluations and reassessments
• Create individualized care plans tailored to each patient’s goals
• Collaborate with nurses, physical therapists, and physicians
• Educate patients and caregivers on proper techniques and safety
• Document patient progress in compliance with state and Medicare guidelines

Qualifications

• Licensed Occupational Therapist in the state of California
• Graduate of an ACOTE‑accredited OT program
• Minimum 1 year of clinical OT experience (Home Health experience preferred)
• Familiar with electronic documentation systems (EMRs)
• Valid driver’s license, auto insurance, and reliable transportation

Pay‑per‑visit: $120 USD – $140 USD
