Overview

The Program Lead oversees a community care team of therapists, including schedule planning, biweekly team meetings, onsite supervision of an ALF therapy program, and providing OT treatments. At Powerback, we provide personalized care to older adults and children, spanning various settings like skilled nursing centers, assisted and independent living facilities, outpatient clinics, and home-based care. Responsibilities

  • Facilitate efficient oversight of rehabilitation services in assigned accounts.
  • Aid in ensuring coordination of services, customer relations, and referral sources.
  • Monitor and maintain the highest standard of rehabilitation services.
  • Coordinate with leadership for support when needed.
  • Assist therapy staff in delivering rehab services effectively and efficiently.
  • Adhere to an established plan of care, department and patient schedule, and timely documentation.
  • Provide direct patient care and routine customer relations touch points.
  • Make referrals to other disciplines or agencies to address comprehensive patient needs.
  • Support the delivery of high-quality rehabilitation services through resource utilization, clinical programs, and staff development.
  • Coordinate Powerback Rehabilitation personnel and consultants within facility or home care contracts.
  • Maintain evidence-based care, adherence to care delivery standards, and practice act compliance.
  • Oversee clinical management of the rehabilitation department, including case management, quality improvement, care planning, clinical utilization, and patient identification.
  • Identify and remove barriers to treatment, seek support when necessary, and assist in coordinating daily staffing and utilization.
  • Ensure financial controls of revenue and expenses in collaboration with the Clinical Operations Area Director.
  • Aid in meeting annual budget goals.
  • Support clinical orientation and ensure a quality orientation process for new rehab staff.
  • Promote Powerback Rehabilitation products and services.
  • Maintain relationships with facility management teams and staff.
  • Ensure clinical management of the rehab department, including oversight related to case management, quality improvement, care planning, clinical utilization, and patient identification.

Qualifications

  • Graduate of an occupational therapy curriculum accredited by AOTA - ACOTE standards, or
  • Bachelor's degree from an occupational therapy curriculum accredited by AOTA - ACOTE standards plus 5 years of progressively responsible OT experience, or
  • Initial registration from the National Board for Certification in Occupational Therapy.
