In this role, you'll deliver personalized occupational therapy to PACE participants in various settings, including our care center, their homes, and virtually. You'll collaborate with an interdisciplinary team (IDT) to create and implement individualized care plans, ensuring each participant's unique needs—whether acute care, restorative support, or ongoing maintenance—are met. As part of the rehabilitation department, you'll partner with our Physical Therapist to enhance the well-being and independence of the elderly adults we serve.

Responsibilities

  • Collaborate with the IDT by completing assessments, care planning, and reviewing service requests to ensure optimal participant outcomes.
  • Provide compassionate occupational therapy to maintain or improve participants' functional abilities, assessing cognitive strengths to tailor personalized care.
  • Evaluate participants and recommend therapeutic exercises that promote physical well-being, adapting plans of care to individual needs.
  • Engage participants and caregivers in setting meaningful goals to empower independence.
  • Deliver treatment following established protocols to ensure high-quality, consistent care.
  • Educate participants and families on enhancing mobility, safety, and independence.
  • Conduct home safety evaluations to create secure environments for prospective participants.
  • Collaborate with the IDT to ensure smooth care transitions between external settings and home.
  • Assist with ordering, inventory, distribution, and maintenance of durable medical equipment.
  • Partner with nursing home, skilled nursing facility, and assisted living staff to address rehabilitation needs.
  • Approve care plans on behalf of the IDT to ensure consistent, thoughtful care.
  • Complete home assessments and deliver treatments in participants' homes.
  • Support overall growth by contributing to additional duties and responsibilities as needed.
  • Actively support SCAN's vision and goals, including exploring AI's potential to transform operations and service delivery.
  • Perform other duties as assigned.

Requirements

  • Graduate of an accredited Occupational Therapy program.
  • Occupational Therapy registration from the National Board of Certification.
  • CPR and First Aid certification (required within the first 60 days of employment).
  • Valid California Occupational Therapy License or proof of eligibility to obtain one.
  • Two (2) years of experience in Occupational Therapy, preferably with frail or elderly populations; PACE program experience is ideal but not required.
  • Valid CA driver’s license, personal transportation, good driving record, and auto insurance as required by State law (for in-home participant visits).
  • Ability to lift, carry, or move up to 50 pounds as needed.
  • Comfort assisting participants with mobility challenges.

This is a 100% onsite position supporting our South Los Angeles market. Your in-person presence is key to building strong team connections and delivering high-quality, coordinated care.

Schedule

Monday through Friday, 8:00 AM to 5:00 PM.
