Occupational Therapist

Responsibilities

• Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients in their homes – conduct patient home visits
• Conduct evaluations and develop occupational therapy treatment plan for our patients
• Deliver treatment/care in accordance with care plan
• Prepare and maintain accurate and complete clinical and progress notes on a timely basis

Qualifications

• Active Occupational Therapist state license
• Valid drivers’ license
• Two or more years of applicable occupational therapist experience
