Occupational Therapist

About the role

Provide personalized, one-on-one Occupational therapy to older adults in private homes or senior living communities, focusing on improving mobility, independence, and quality of life through evidence-based treatments. Perform evaluations, develop care plans, and educate patients, caregivers, and staff to promote safety and aging in place. Utilize telehealth when appropriate to enhance care access and flexibility. Collaborate with interdisciplinary teams including PTs, SLPs, and fitness specialists. Maintain accurate documentation within electronic medical records and adhere to regulatory standards.

Requirements

• Valid OT license or eligibility in the practice state
• Degree from an accredited OT program
• Proficiency in computer and EMR systems
• Current CPR certification

Physical and other requirements

• Ability to lift 10–50 lbs, assist with patient transfers, and perform therapeutic activities
• Capability to drive and navigate various environments
• Wearing PPE as needed
