Responsibilities
- Provide evidence-based evaluation and treatment to diverse patient populations
- Develop individualized care plans
- Deliver interventions across the continuum of care
- Collaborate closely with physicians, nurses, rehab professionals, and families to ensure comprehensive patient care
- Maintain thorough documentation in EMR
- Contribute to quality improvement initiatives
