About the Facility

Ahava Therapy Services is hiring a Full-Time Occupational Therapist for Henderson Health & Rehab, a 137-bed skilled nursing facility that provides short- and long-term care and outpatient services. Therapists at Henderson work as part of an in-house collaborative rehab team, with strong leadership support, to help residents achieve independence and optimal function.

Responsibilities

  • Review clinical information and evaluate its impact on the assessment and treatment process.
  • Conduct comprehensive functional assessments using standardized procedures.
  • Develop and implement individualized treatment plans with achievable functional goals.
  • Document evaluations, daily treatments, progress notes, and discharge summaries accurately and promptly.
  • Rewrite treatment plans based on patient performance and progress.
  • Supervise and guide therapy assistants, ensuring proper treatment techniques and the delivery of care.
  • Educate residents, families, and caregivers about exercises and therapeutic procedures to support carryover.
  • Collaborate with nursing staff, interdisciplinary team members, and families to maximize functional outcomes.
  • Assist with discharge planning and maintain knowledge of community resources.
  • Participate in care conferences, meetings, and quality improvement initiatives.
  • Maintain clinical competence through continuing education opportunities.
  • Adhere to all regulatory, professional, and facility standards, including HIPAA and payer guidelines.
  • Support the company’s mission, vision, and professional expectations.
  • Maintain expected productivity levels and demonstrate excellent teamwork and communication skills.

Qualifications

  • Bachelor’s or Master’s Degree in Occupational Therapy
  • Active OT license in the state of practice
  • BLS/CPR certification (current or obtained during orientation)
  • Strong clinical reasoning, communication, and documentation skills
  • Ability to collaborate effectively within an interdisciplinary team
