About the role
Sea Mar Community Health Centers is a Federally Qualified Health Center (FQHC) founded in 1978, serving diverse communities, with a focus on Latinos in Washington State. As a mandatory COVID-19 and flu vaccine organization, we are recruiting for the Full-Time OB Coordinator position at our Medical Clinic in Burien, WA.
Responsibilities
- Carry out the primary functions of OB tracking, from pregnancy confirmation to postpartum support
- Participate in daily huddles, identify patient needs, and provide point of care services and education
- Work closely with patients and clinical team members to ensure clinical measures are addressed
- Enhance quality and patient-centered care at Sea Mar Community Health Centers and integrate with other services as needed
- Provide excellent customer service by using active listening skills and greeting patients professionally
- Make patients a priority, assisting them to meet their individual needs
- Perform prenatal intake and obtain past medical history
- Document intake information in the EPIC system
- Update and provide information on shared OB and MSS clients as needed
- Aid patients with pregnancy management and connect them to social support services
- Assist with scheduling, referrals, and tracking, ensuring patient attendance
- Complete prenatal contracts and paperwork, provide orientation, and send follow-up letters
- Advocate for patient services with community, social service, and medical providers
- Connect patients to Sea Mar and non-Sea Mar resources and track available services
- Track patient adherence with their plan of care and communicate outcomes to providers
- Prepare monthly reports for provider meetings and updated reports of the prenatal panel
- Ensure the OB Database is updated regularly
- Perform other duties as assigned by supervisors
Qualifications
- Ability to work independently, prioritize workload, and meet deadlines
- Critical thinking skills and confidentiality maintenance
- Excellent organizational skills and ability to handle multiple tasks simultaneously
- Knowledge of medical terminology and/or behavioral health topics
- Strong decision-making and prioritization skills
- Respectful, professional, and empathetic communication skills
- Knowledge of the Patient-Centered Medical Home Model and motivational interviewing skills
- Bilingual English/Spanish required
