Responsibilities
- Complete quarterly visits, observation, monitoring, and monthly contacts.
- Conduct iHC assessments according to OAAS timelines and enter them into the data system within 10 days.
- Conduct and complete annual Comprehensive Plan of Care (CPOC) according to OAAS timelines.
- Submit plans to the Support Coordinator Supervisor for approval according to OAAS and ESL timelines.
- Plan, schedule, and coordinate quarterly meetings with participants and providers.
- Respond to participant changes in needs, completing and submitting appropriate paperwork when revisions are needed.
- Research, access, and link services for participants.
- Communicate and work with providers to ensure appropriate service provision.
- Maintain all responsibilities and requirements regarding critical incidents as required by OAAS.
- Meet with supervisor for weekly supervision, consultation, and participant needs.
- Attend and participate in weekly staff meetings and required training.
- Act as a mandated reporter, making reports immediately upon learning of incidents of abuse or neglect to Adult Protective Services and Elderly Protective Services.
- Assess and address needs expressed by participants, their families, or guardians, and professionals in the CPOC.
- Make referrals to appropriate agencies and providers, documenting and following up promptly.
- Establish ongoing rapport with participants and assist them in maintaining community living.
- Respond promptly to participant referrals and needs.
- Empower participants and families to be active participants in their care.
- Maintain an active caseload not to exceed 35 participants.
- Maintain a 40-hour work week, with at least 50% of time during normal business hours.
- Work with participants, service providers, and families to set and meet long-range and short-range goals.
- Report consultation needs regarding participant issues to the Support Coordinator Supervisor.
- Serve as Long Term Care Support Coordinator for assigned NOW Waiver, EPSDT, Children’s Choice, Supports Waiver, and other contracted populations.
- Keep accurate and up-to-date records and documentation.
- Complete CPOC, quarterly reviews, amendments, and other required paperwork in cooperation with the Support Coordination Team.
- Provide needed assistance or fill in for other Support Coordinators as directed by the supervisor.
- Participate in public relations activities to orient and market program services to the public, including Easter Seals activities if requested.
- Provide input and assistance in preparing program reports and state-required reports.
- Work with supervisors and project managers to ensure communication and coordination with other Easter Seals staff.
- Evaluate program effectiveness and compliance with state/contract requirements and program goals.
- Participate in staff meetings to maintain open lines of communication.
- Participate in state Support Coordination activities and training opportunities as budget/need allows.
- Complete at least 40 hours of training per year.
Requirements
- Bachelor’s Degree in Social Services, Human Services, or a related field.
- Own reliable transportation and willingness to travel during the day to outlying parishes as required.
- Ability to multi-task and meet deadlines.
- Excellent written and verbal communication skills.
- Proficiency in Microsoft Office Software (Word, Excel) and general computer literacy.
- Team-player mindset.

Physical Requirements
- Regularly operate a computer, file and retrieve written documents, and communicate with others on the phone and in-person.
- Frequently walk, sit, use hands, and lift or move lightweight items.
- Ability to move independently or with reasonable accommodation within the facility and community.
- Must be able to travel and meet with participants.
Working Conditions
Work is performed in a normal office setting that is environmentally controlled and out in the field.
