ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Supervision and management
  • Developing strong internal and external partnerships
  • Supervise and support the nursing staff, including RNs, LPNs, and CNAs
  • Convenes and facilitates weekly team meetings to review patient progress and for discharge planning
  • Works in collaboration with social services (case manager/behavioral health)
  • Aid in the development and enforcement of clinical protocols in collaboration with the Director of Nursing and Medical Director
  • Aid in the development and implementation of all medication storage and management protocols
  • Manage inventory and purchasing of nursing/medical supplies
  • Observe HIPAA requirements and provide patient privacy within the limitations of the site
  • Provide medication management using specified guidelines outlined by the medical respite standard in accordance with the medical respite standards
  • Support clinical providers and social services staff on regular care, admissions, and discharge goal plans
  • Review medical respite criteria for admissions with referring entities
  • Update and maintain daily census
  • Assist with patients’ transportation
  • Assure the safety of all staff and patients
  • Coordinate with nursing staff and the Respite Program Administrator for the scheduling of medical appointments
  • Aid in the coordination of Medication-Assisted Therapy, IV antibiotics, oxygen, wound care, and other more specialized clinical patient care needs
  • Participate in professional trainings, webinars, etc.
  • Maintain knowledge of programs offered and take responsibility for keeping current with respite program changes and new services offered
  • Assume new responsibilities as new needs are identified
  • Infection Control/Emergency Preparedness: Provide coordination and monitoring services for reporting of suspected or confirmed communicable diseases that can be transmitted through routine contact, and provide on-call services for all respite services in partnership with the Respite Program Administrator
  • Monitor respites for consistent hand washing, care of the environment, use of safety syringes and sharps disposal, and use of personal protective equipment
  • Manage or participate, as needed, in respite-based communicable disease outbreaks, emergency preparedness/disaster response, and other health initiatives
  • Provide education to the community and academic health care professionals to provide comprehensive, culturally competent, continuous care to people experiencing homelessness
  • Provides monthly tracking and record keeping on-site for the environment of care report monthly; and discuss and monitor remediation of any deficiencies with PHMC Health leadership staff
  • Clinical: Provide resources for screening and immunization to respite staff, as indicated
  • Document all services promptly
  • Reviews, records, and submits monthly CLIA lab reports
  • Oversees seasonal influenza vaccine process in collaboration with site nurse(s)
  • Maintains site sign requirements to meet Joint Commission requirements
  • Maintains site equipment maintenance
  • Attends UDS measures meetings as requested
  • Supports effective utilization of the Electronic Medical Record for patient care
  • Conducts auditing of nursing documentation; provides feedback and corrections
  • Routinely monitor and report outcomes based on HCH data, MARS, on-site logs, and chart review, and develop action plans to address issues identified

JOB REQUIREMENTS

  • Good written and verbal communication skills
  • Sensitivity to the needs and issues of diverse and disenfranchised individuals
  • Strong leadership
  • Five years in the nursing field
  • Infection control and wound care experience required
  • Supervisory experience required
  • Experience in community health nursing, preferred
  • Experience with providing care in shelter and haven environments preferred
  • Emergency preparedness experience preferred
  • Refined organizational skills and leadership qualities
  • Bachelor of Science in Nursing (BSN) or Master of Science in Nursing (MSN) preferred
  • PA RN license
