About the Role
The Nursing Services Supervisor oversees day-to-day operations of their designated region to ensure agency goals and superior patient care are achieved. This role manages and directs a team of clinicians, including RN Case Managers, RNs, LPNs, Social Workers, and Home Health Aides, to provide safe, effective, and appropriate home care services.
Responsibilities
- Act as the primary resource for clinician questions and concerns, providing guidance on proper patient care.
- Conduct regular individual meetings with clinicians to address specific issues and provide guidance.
- Collaborate with the QA department to educate staff on OASIS corrections and chart revisions.
- Direct teams to ensure client needs are met according to agency policies and procedures.
- Educate clinicians on clinical services, policies, and procedures as needed.
- Serve as the clinical expert on home health guidelines and Medicare regulations.
- Ensure accuracy, completeness, and timeliness of clinical documentation in compliance with agency policies, regulatory requirements, and industry standards.
- Perform weekly reviews of clinician productivity and report to the Area Manager.
- Coordinate 24-hour on-call duties and act as a backup to provide patient care if needed.
- Review on-call logs and summarize reasons for calls and resolutions.
- Lead weekly case conferences to review admissions, discharges, re-certifications, potential LUPAs, patient outcomes, team SHP scoring, and other patient-specific needs.
- Ensure discharge planning is completed and required documentation is assigned or returned for upload to the chart.
- Conduct ongoing chart and clinician reviews, including:
- New admission reviews
- Frequency orders
- Scheduled supervisory visits
- Wound care
- Visit exceptions (overlapping visits and visits less than 15 minutes)
- Outstanding visits (past due visits)
- Mileage (review for excessive mileage)
- Conduct ongoing nurse meetings to provide in-services and promote educational training based on the Education Plan and regional skills needs.
Personnel Management
- Review employee timecards/payroll daily via HRIS to monitor overtime, pay premiums, on-call, absenteeism, and ETO requests, ensuring adequate patient coverage.
- Collaborate with human resources on policy/procedure violations or performance transgressions to deliver performance improvement plans and corrective actions.
- Assist in screening and interviewing new agency personnel and make recommendations to the Director of Clinical Operations and Human Resources.
- Assist in new hire orientation and ensure precepting is completed to agency expectations.
- Perform annual observation visits with clinicians to assess competency and address educational and developmental needs.
- Complete performance appraisals in a timely manner to deliver effective feedback.
Staffing Oversight
- Perform staffing oversight to ensure adequate patient coverage in the designated region.
- Promote self-scheduling for field staff and perform high-level overviews to plan for productivity.
- Prioritize referrals based on acuity and timely initiation of care requirements.
- Collaborate with the Area Manager to ensure territories are assigned appropriately for adequate staffing coverage.
- Perform screening calls to hospital and physician office referrals to confirm patient availability and appropriateness for admission.
- Collaborate with intake regarding scheduling of new admissions and evaluations, maintaining consistent communication with the Intake department, marketing department, and Area Manager regarding staff availability.
- Partner with the Area Manager regarding staff vacations and call-ins to ensure patients are seen in compliance with physician-ordered frequency.
- Collaborate with the intake department daily to review staffing needs and assist clinicians with self-scheduling as needed.
Direct Patient Care
- Provide skilled nursing services in accordance with adult and geriatric patients' plans of care, including comprehensive evaluations, ongoing monitoring, and education/training for patients, caregivers, and family members.
- Complete initial and ongoing assessments to establish an individualized home health plan of care with the physician.
- Administer nursing procedures and treatments as indicated by the physician to achieve desired patient outcomes.
- Communicate with the physician regarding patient needs, report changes in patient condition, and obtain physician orders for revisions to the plan of care.
