Nexus Family Healing is seeking a Nursing Manager for our PRTF program in Portland, Oregon. This small, secure program serves up to six youth at a time, ages 10-17, with mental and developmental disabilities who have become involved in the justice system. Located at Nexus’ Walden Crossing, the program provides high staff-to-youth ratios, 24/7 nursing, recreational and therapeutic professionals, psychiatric services, and an array of therapies.

About the Role

Our Nursing Manager oversees the provision of medical services that promote resident physical health and well-being, ensuring related staff training aligns with licensing guidelines.

Responsibilities

  • Medical Service Delivery
    • Coordinates medical aspects of admissions and ensures completion of annual physical exams; arranges necessary follow-up appointments and/or referrals.
    • Oversees the nursing department’s delivery of medical services to the resident population in partnership with the program team.
    • Coordinates special needs appointments with external physicians and clinics.
    • Collects and assesses residents’ medical data systematically, ensuring medical records are current.
    • Monitors acute and chronic medical conditions; provides treatment or consultation as needed and communicates with physicians.
    • Gathers relevant information and provides appropriate care in emergency situations, directing others as necessary.
  • Treatment Team Collaboration
    • Serves as a resource to staff regarding resident medical issues.
    • Participates in new client diagnostic meetings to ensure cohesive individual diagnoses are reflected in clients’ Individual Treatment Plans (ITP).
    • Implements, monitors, and ensures compliance of all medical service documentation.
    • Reviews and assesses Medication Administration Records (MARs) regularly.
    • Supervises medication administration and documentation; reports side effects to physicians and documentation errors to management.
    • Coordinates and/or provides staff training in medication administration and infection control.
    • Monitors prescription and medical supplies, ensuring timely orders.
    • Tracks staff Hepatitis vaccines per contracting agency guidelines.
    • Provides first aid care and consultation for work-related employee injuries.
  • Employee and Talent Management
    • Supervises and participates in staff orientation, mentorship, training, and development within Nexus and licensing guidelines.
    • Leads, coaches, and develops staff, providing training on resident issues, treatment plans, behavior management, Medicaid compliance, and documentation.
    • Provides ongoing performance management and conducts timely, constructive evaluations with development opportunities.
    • Consults with Human Resources on employee performance issues.
    • Builds cohesive teams by communicating clear missions, outlining goals, and promoting shared accountability.
    • Identifies and addresses barriers to team development, using facilitation strategies to achieve goals.
    • Enforces policies and holds staff accountable.
  • Nursing Program and Internship Coordination
    • Sources and recruits top talent from local colleges and universities.
    • Develops an on-site curriculum for the Nurse rotational program.
    • Supervises and facilitates continuous education for the internship program.
    • Provides feedback and updates to colleges regarding the Nurse rotational program.

Requirements

  • Graduate of an accredited school of nursing.
  • Valid Registered Nurse license.
  • Minimum of 5 years related experience, preferably in a residential setting.
  • First Aid/CPR/CPI/Infection Control certification.
  • Valid driver’s license; must meet state and Home Office driving requirements.

Preferred Qualifications

  • 2 years of supervisory experience.

Pay

$115,000–$125,000 annually.

Schedule

  • Full-time, weekday schedule.
  • Intermittent on-call/after-hours availability for phone support.
