Nursing Case Manager

About the role

D&S Community Services, a part of the Sevita family, seeks a Licensed Practical Nurse (LPN) Case Manager to join our mission-driven team. This role focuses on developing and implementing care plans for IDD clients, ensuring services align with licensing guidelines, medical standards, and agency policies.

Responsibilities

• Performs client intake assessments and submits required documentation for prior authorization of client services.
• Develops a plan of care based on nursing diagnoses, client input, and physician orders, including medical interventions and measurable goals.
• Routinely reviews client medical data to assess the effectiveness of services.
• Investigates and addresses client or employee concerns, ensuring proper training and supervision of aides.
• Coordinates client home care services through timely documentation and transfer of medical data to physicians and other caregivers.
• Provides input for policy development and participates in quality assurance committees.

Requirements

• Bachelor’s Degree in Nursing
• At least two years’ experience as a LPN
• Valid nursing license in state of work

Qualifications
