Nursing Case Manager

About the role

The Nursing Case Manager position at NeuroRestorative is available in a variety of locations and community-based settings. This role is not virtual and requires on-site work. The nurse will manage up to 20 individuals in four group homes, focusing on those with Intellectual Disability (ID)/DD, Autism, or Traumatic Brain Injury (TBI).

Responsibilities

• Making appointments, conducting med. Cart audits, and ordering medications
• On-call every other week for emergency situations
• Creating and implementing protocols such as seizure and fall management
• Monthly reporting and staff training
• Ensuring proper aide orientation and training
• Providing instruction to individuals, family members, and primary caregivers
• Timely completion of required documentation and transfer of medical data to physicians and therapists
• Participation in advisory boards and quality assurance committees

Qualifications

• Bachelor’s Degree in Nursing with a valid state nursing license
• At least two years of relevant experience
• Self-motivated and detail-oriented with excellent communication and analytical skills
