Nurse Case Manager

Responsibilities
- Provide care coordination services to patients in collaboration with interdisciplinary teams.
- Assess patient needs and develop care plans to ensure continuity of care.
- Facilitate communication between patients, families, and healthcare providers to optimize patient outcomes.
- Manage patient transitions between hospital, home, and community settings.
- Collaborate with other departments to ensure seamless care delivery.
- Ensure compliance with regulatory standards and policies.
Requirements
- Bachelor of Science in Nursing (BSN) or Bachelor of Social Work (BSW) preferred.
- NYS Licensure as RN required.
- LMSW or LCSW preferred.
- Case management certification preferred.
- 3 years of RN experience in an acute care setting.
- Ledger experience preferred.
- Utilization review, discharge planning, or case management experience preferred.
Qualifications
- Strong interpersonal and communication skills.
- Ability to work independently and as part of a team.
- Proficiency in Microsoft Office Suite.
- Knowledge of healthcare regulations and policies.

Pay

Schedule
This is a Monday through Friday position with rotating weekends. The standard hours are 8am-4pm.
