Nurse Case Manager

Responsibilities

• Provide care coordination services to patients in collaboration with interdisciplinary teams.
• Assess patient needs and develop care plans to ensure continuity of care.
• Facilitate communication between patients, families, and healthcare providers to optimize patient outcomes.
• Manage patient transitions between hospital, home, and community settings.
• Collaborate with other departments to ensure seamless care delivery.
• Ensure compliance with regulatory standards and policies.

Requirements

• Bachelor of Science in Nursing (BSN) or Bachelor of Social Work (BSW) preferred.
• NYS Licensure as RN required.
• LMSW or LCSW preferred.
• Case management certification preferred.
• 3 years of RN experience in an acute care setting.
• Ledger experience preferred.
• Utilization review, discharge planning, or case management experience preferred.

• Ability to work independently and as part of a team.
• Proficiency in Microsoft Office Suite.
• Knowledge of healthcare regulations and policies.

Pay

Schedule

This is a Monday through Friday position with rotating weekends. The standard hours are 8am-4pm.
