About the role

Develop and implement personalized care plans for individuals with intellectual and developmental disabilities, ensuring high-quality, person-centered care in a mission-driven environment.

Responsibilities

• Conduct client intake assessments
• Coordinate services and review medical data
• Supervise aides and educate clients and caregivers to optimize client functioning
• Document care and participate in quality assurance activities
• Travel as needed
• Perform physically demanding tasks, including heavy work
• Adhere to agency policies and fulfill on-call responsibilities

Requirements

• Bachelor’s Degree in Nursing
• Valid nursing license
• At least two years of experience
