Coordinate and manage person-centered care for individuals receiving home and community-based services. Complete assessments, develop and monitor care plans, and collaborate with an interdisciplinary team to ensure safe and effective services. Connect individuals and families with community resources, advocate for their needs, and promote independence and quality of life.
