Job Summary
The Nurse supports and assists in providing holistic, therapeutic, person-centered care for custodial, post-acute, sub-acute, and memory care residents. The Nurse will achieve and maintain the highest quality of life for each resident residing in or under the care of the facility. Our RN's and LPN's collaborate with our CNA's who provide ADL care for our residents. Nurses guide the care and work as a team with their CNA's and other support staff to provide weights, vitals, skin care and change in resident behavior/status information.
Responsibilities
- Supports holistic, therapeutic, person-centered care for residents
- Collaborates with CNA's to provide ADL care
- Guides care and works as a team with CNA's and other support staff
- Provides weights, vitals, skin care, and changes in resident behavior/status information
- Participates in clinical assessments/data gathering and care
- Awareness of admission/discharge details
- Medication management
- Engages in Quality Assurance Performance Improvement process
Requirements
- High School Diploma or GED
- Current Registered Nurse (RN) license in the state of Illinois or Wisconsin
- Current Licensed Practical Nurse (LPN) license in the state of Illinois or Wisconsin
- Experience in long-term care or similar setting preferred
- Ability to lift/push/pull 35 pounds and carry it 35 feet
- Pass a healthcare worker background check and a drug test
Qualifications
- Fluent in English
- Knowledge of Point Click Care electronic medical record system
- Ability to work flexible hours to accommodate various schedules
Skills
- Strong communication and interpersonal skills
- Effective problem-solving and decision-making abilities
- Ability to work independently and as part of a team
- Proficient in Microsoft Office applications

Pay
Salary starting at $36/hr for LPN's and $40/hr for RN's
