Nurse Practitioner - Wound Care

Provides direct and indirect nursing care by utilizing advanced scientific knowledge, nursing theory, and skills to assess, plan, and implement appropriate health and nursing care to patients. Serves as a consultant and resource for advanced nursing knowledge, guiding and evaluating care provided by other healthcare team members. Teaches, counsels, and develops educational programs for individuals, groups, and communities.

Responsibilities

• Evaluates physical and psychosocial health status through comprehensive health history, physical examination, and diagnostic procedures.
• Assesses normal and abnormal findings from history, physical examination, and diagnostic reports to diagnose health status and issues.
• Oversees and directs nursing, medical, and clerical personnel in provision of patient care.
• Plans, implements, and evaluates patient care, managing prescribed plans of care.
• Consults with patients, support systems, and healthcare team members to provide or refer ongoing care.
• Initiates and maintains accurate records, legal documents, and care reports.
• Develops individualized teaching plans and counsels individuals, families, and groups on health and illness, promoting health maintenance.
• Designs and implements community educational programs targeting health promotion and illness prevention.
• Participates in periodic and joint evaluation of services rendered.
• Conducts research and analyzes health needs of populations, designing programs for at-risk groups.
• Participates in policy analysis and development of new initiatives within the specialty area.
• Contributes to the development and improvement of healthcare delivery systems to enhance quality and consumer access.
• Incorporates Kaiser Permanente Nursing Vision, Model, and Values into practice.
• Performs other duties as assigned and may provide direct patient care to maintain clinical expertise and competency.

Qualifications

• Master’s degree in nursing in a relevant field of specialty.
• Advanced Practice Registered Nurse License (Hawaii).
• Registered Nurse License (Hawaii).
• National Provider Identifier.
• Basic Life Support certification from the American Heart Association.
• Nurse Practitioner National Certificate.

Skills

• Demonstrated knowledge and skill in customer service, conflict resolution, counseling, problem solving, decision making, oral and written communication, interpersonal relations, adaptability, change management, results orientation, and teamwork.
• Preferred: Experience as a Nurse Practitioner in the specialty area.
• Preferred: Knowledge of word processing, spreadsheet, and database PC applications.
• Preferred: Valid Hawaii APRN Recognition with Prescriptive Authority (must meet education requirements for Hawaii state APRN prescriptive authority).

Schedule

• Part-time, 24 scheduled weekly hours.
• Variable shift: Monday through Sunday.
• Working hours: 07:00 AM to 06:00 PM.
• Travel required: Yes, up to 50% of the time.
• Work setting: Onsite.

Pay

$74.47 - $78.39 per hour.
