About the role
The Family Nurse Practitioner ensures delivery of comprehensive health care in accordance with the philosophy, standard of practice, objectives and policies of The Institute for Family Health. The Nurse Practitioner is responsible for diagnosis and treatment of common and chronic illness, focusing on health promotion and disease prevention activities including patient education and counseling.
Responsibilities
- Perform health assessment interviews and complete physical assessments
- Order and interpret diagnostic tests
- Develop and implement individualized plans of care based on diagnosis and client needs
- Prescribe medications and treatments as appropriate
- Collaborate with the health care team and make referrals to other disciplines as indicated
- Record, report, and interpret client responses to the plan of care
- Teach, counsel, and assist individuals and families in assuming responsibility for illness prevention and health promotion, maintenance, and restoration
- Collaborate with team members to provide regular health screening and other health promotion measures
- Identify and facilitate access to community resources for clients
- Conduct home or hospital visits and respond to calls as appropriate
- Provide age-appropriate care based on physical, psychological, educational, safety, and related criteria
- Provide telephone coverage as directed by supervisor
- Provide feedback to Nurse Manager, Practice Administrator, and/or Medical Director regarding policies, practices, and individual performance
- Modify approaches based on patient age-specific needs and responses to treatment
- Use age-appropriate communication techniques
Requirements
- Bachelor’s degree and master’s degree in nursing
- Graduate of an accredited Nurse Practitioner program
- Bilingual, Spanish preferred
- Current NY State Registered Nurse License and Registration
- Nurse Practitioner Certification and Registration
- Current Practice Agreement
- Certified or eligible for certification by recognized specialty organization
- Current BCLS and Infection Control certification
- Current Child Abuse certification
