Responsibilities
- Educate and mentor Nurse Practitioners providing home-based primary and palliative care to complex, home-limited patients.
- Onboard new NPs and deliver hands-on clinical training in patient homes.
- Reinforce best practices in assessment, chronic disease management, telehealth, and care coordination.
- Partner with an interdisciplinary team to refine training materials and support quality initiatives.
- Ensure NPs deliver compassionate, compliant, and outcome-driven care aligned with organizational mission.
Qualifications & Skills
- Strong clinical knowledge in primary and palliative care.
- Experience in training or mentoring healthcare professionals.
- Excellent communication and interpersonal skills.
- Ability to work independently and collaboratively in a home-based setting.
- Knowledge of telehealth practices and care coordination strategies.
