As a Nurse Practitioner (NP) or Physician Assistant (PA) at Conviva Senior Primary Care, you will be part of a collaborative, multidisciplinary team that includes physicians, pharmacists, care coaches, behavioral health specialists, and more—all working together to deliver personalized, high-quality care. In this role, you will work onsite at our Med Center and Boerne clinics located in the San Antonio area. You will report to our Associate Medical Director.

Your role & responsibilities
- Provide comprehensive care to seniors following clinical standards.
- Collaborate daily with your care team to ensure coordinated, high-quality care.
- Manage referrals, hospital/SNF coordination, DME, and home health services.
- Document care accurately and efficiently with support from quality-based coders.
- Participate in on-call rotation and support growth plans at your center.
- Demonstrate passion for providing value-based care.
Required
- Master’s Degree in Nursing or completion of a PA program with board certification.
- Active, unrestricted NP or PA license in the state of practice.
- If selected, required to be screened for TB.
Preferred
- 2 or more years of experience in value-based care, primary care, or Medicare Advantage settings.
- Active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and/or Medicare/Medicaid numbers.

Pay range
The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
- $123,800 – $170,300 per year
This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
