WellBe Senior Medical delivers value‑based primary care directly in patients’ homes, replacing profit‑driven volume with autonomous, interdisciplinary teamwork. Responsibilities

• Conduct comprehensive primary care visits and physical exams in patients’ homes.
• Create longitudinal care plans for chronic disease management.
• Collaborate with an interdisciplinary team to ensure seamless care transitions.
• Treat acute needs in the home to prevent emergency room visits.

Requirements

• Master’s degree (NP) with a focus on Acute Care, Adult, Geriatric, and/or Family.
• Active, unencumbered NP license and Board Certification (ANCC or AANP) in West Virginia.
• Valid driver’s license and reliable vehicle for local field‑based travel.

Pay

Salary range: $110,000 – $156,000 per year.
