Responsibilities
- Provides direct patient care and support to multidisciplinary health team working within the scope of practice of a Nurse Practitioner (NP).
- Collaborates with a multidisciplinary healthcare team to ensure high-quality, evidence-based outcomes.
- Develops comprehensive care plans, coordinates patient care, delivers ongoing treatment while fostering a strong patient-provider relationship.
- Ensures adherence to clinical protocols and standards of care.
- Maintains program excellence and professional growth through ongoing education and skill development.
- Participates in interdisciplinary team meetings to discuss patient cases and develop care plans.
- Provides patient education and counseling on various health topics.
- Monitors patient progress and adjusts care plans as necessary based on clinical assessment and feedback from patients and other healthcare providers.
- Collaborates with other healthcare professionals to optimize resource utilization and enhance patient experiences.
- Engages in quality improvement initiatives to enhance patient care and satisfaction.
- May provide leadership and mentorship to less experienced healthcare professionals as appropriate.
Requirements
- Current licensure as a Nurse Practitioner in the state where the position is located.
- At least 2 years of relevant clinical experience in a healthcare setting.
- Experience in providing direct patient care and collaborating with a multidisciplinary healthcare team.
- Strong communication and interpersonal skills to build and maintain effective relationships with patients, families, and colleagues.
- Ability to work independently and as part of a team, demonstrating initiative and adaptability.
- Proficiency in electronic health record systems and other relevant software tools.
- Knowledge of best practices in patient care, including evidence-based medicine and patient-centered care.
- Ability to manage time effectively and prioritize tasks to meet deadlines.
- Passion for improving patient outcomes through continuous learning and innovation.
Qualifications
- Master's degree in Nursing (MSN) or related field.
- Advanced certification as a Nurse Practitioner (e.g., Family Nurse Practitioner, Adult-Gerontology Acute Care NP).
- Current licensure as a Nurse Practitioner in the state where the position is located.
- Experience in providing direct patient care and collaborating with a multidisciplinary healthcare team.
- Strong communication and interpersonal skills to build and maintain effective relationships with patients, families, and colleagues.
