Responsibilities

  • Provides direct patient care and support to multidisciplinary health team working within the scope of practice of a Nurse Practitioner (NP).
  • Collaborates with a multidisciplinary healthcare team to ensure high-quality, evidence-based outcomes.
  • Develops comprehensive care plans, coordinates patient care, delivers ongoing treatment while fostering a strong patient-provider relationship.
  • Ensures adherence to clinical protocols and standards of care.
  • Maintains program excellence and professional growth through ongoing education and skill development.
  • Participates in interdisciplinary team meetings to discuss patient cases and develop care plans.
  • Provides patient education and counseling on various health topics.
  • Monitors patient progress and adjusts care plans as necessary based on clinical assessment and feedback from patients and other healthcare providers.
  • Collaborates with other healthcare professionals to optimize resource utilization and enhance patient experiences.
  • Engages in quality improvement initiatives to enhance patient care and satisfaction.
  • May provide leadership and mentorship to less experienced healthcare professionals as appropriate.

Requirements

  • Current licensure as a Nurse Practitioner in the state where the position is located.
  • At least 2 years of relevant clinical experience in a healthcare setting.
  • Experience in providing direct patient care and collaborating with a multidisciplinary healthcare team.
  • Strong communication and interpersonal skills to build and maintain effective relationships with patients, families, and colleagues.
  • Ability to work independently and as part of a team, demonstrating initiative and adaptability.
  • Proficiency in electronic health record systems and other relevant software tools.
  • Knowledge of best practices in patient care, including evidence-based medicine and patient-centered care.
  • Ability to manage time effectively and prioritize tasks to meet deadlines.
  • Passion for improving patient outcomes through continuous learning and innovation.

Qualifications

  • Master's degree in Nursing (MSN) or related field.
  • Advanced certification as a Nurse Practitioner (e.g., Family Nurse Practitioner, Adult-Gerontology Acute Care NP).
  • Current licensure as a Nurse Practitioner in the state where the position is located.
  • Experience in providing direct patient care and collaborating with a multidisciplinary healthcare team.
  • Strong communication and interpersonal skills to build and maintain effective relationships with patients, families, and colleagues.
  • Ability to work independently and as part of a team, demonstrating initiative and adaptability.
