Your Impact

• Deliver field‑based care: conduct comprehensive primary care visits and physical exams in patients’ homes.

• Prevent ER visits: treat acute needs in the home to keep seniors safe, healthy, and out of the hospital.

Job Requirements

• Advanced Degree: Master’s Degree (NP) with a preferred focus on Adult, Geriatric, Acute Care and/or Family.
• State Required Credentials: active, unencumbered NP license and Board Certification (ANCC or AANP).
• Reliable Mobility: valid driver’s license and vehicle for local field‑based travel.
• Legal authorization to work in the United States without employer sponsorship now or in the future.
• Pre‑employment drug screening and background check required.

Pay

Salary range: $110,000 – $175,000 plus mileage and full benefits. Relocation assistance available.
