About the Role
The Advanced Health Provider (AHP) serves as a team leader in our patient care model, which includes a multi-faceted treatment team. This role involves attending patients, conducting medication management sessions, psychiatric evaluations, creating treatment plans, and participating in various meetings and evaluations.
Responsibilities
- Attend patients and assist with patient coverage at assigned locations, including multiple levels of care or virtual services.
- Conduct medication management sessions and psychiatric evaluations.
- Create patient treatment plans and manage updates, conduct patient and family education, and participate in team meetings.
- Create discharge plans, participate in the call schedule, complete charting and documentation within 24 hours, round on patients as required, and perform medication education with nursing staff.
- Inspire treatment team members, participate in quality assurance and performance improvement activities, and identify new program development opportunities.
- Advocate for the organization, provide support and education to patients and employees, and communicate effectively with referring parties and payors.
- Support admissions by providing initial placement recommendations based on admitting criteria, clinical parameters, and medical history, and communicate with other departments and agencies as needed.
- Ensure adherence to employer policy and EMTALA guidelines during the admission process.
Requirements
- Licensed Nurse Practitioner in the state.
- Board-eligible or board-certified.
- Graduated from an accredited college or university.
- Completed required practical training as per educational institution and licensing body.
- Two or more years of experience working as a provider in another licensed hospital or clinic, preferred.
- Leadership and/or supervisory experience in a behavioral health organization is preferred.
- Experience with process improvement implementation preferred.
