Nurse Practitioner - Psychiatric Mental Health

Key Responsibilities

• Conduct psychiatric evaluations and follow-up visits.
• Diagnose and manage mental health conditions.
• Develop and monitor individualized treatment plans.
• Manage psychiatric medications as appropriate.
• Collaborate with facility staff and other healthcare providers.
• Maintain accurate clinical documentation and patient records.
• Provide compassionate, patient-centered care to the SNF/LTC population.

Requirements

• Active PMHNP/APRN licensure required.
• Psychiatric/behavioral health experience is required.
• SNF/LTC experience is preferred but not mandatory.
• Strong clinical, communication, and patient-management skills.
• Able to work independently and collaborate with facility-based care teams.

Ideal Candidate

An experienced PMHNP who is comfortable providing psychiatric care to an older/adult population and is interested in working in a Skilled Nursing/Long-Term Care environment. Experience with SNF/LTC patients is a strong plus.
