Nurse Practitioner/Physician Assistant

Total Extended Care Services, a growing Physical Medicine and Rehab group, is seeking a dedicated Nurse Practitioner or Physician Assistant to partner with our PM&R physician in the sub-acute rehab setting. This role is ideal for those passionate about improving patient outcomes within an interdisciplinary, specialized team.

About the Role

• Location: In-person at skilled nursing facilities in Michigan
• Schedule: Flexible options available for full-time and part-time positions
• Culture: Detail-oriented, outcome-focused, stable, people-oriented, and team-driven

Responsibilities

• Partner with the facility’s therapy department to deliver optimal rehab care
• Communicate and collaborate with the patient’s primary care team to ensure the best outcomes
• Complete timely charting and documentation (can be done from home)
• Provide on-site pain management with a focus on reducing re-admission rates
• Coordinate care with medical providers, therapists, social workers, nurses, and families

Requirements

• Licensed Nurse Practitioner or Certified Nurse Practitioner (Required)
• Licensed Physician Assistant or Certified Physician Assistant (Required)
• Must be licensed in Michigan
• Reliable transportation
