Total Extended Care Services, a growing Physical Medicine and Rehab group, is looking for a dedicated Nurse Practitioner or Physician Assistant to partner with our PM&R physician in the sub-acute rehab setting. This position is ideal for those passionate about improving patient outcomes and working within an interdisciplinary, specialized team.
About the Role
Location: In-person at skilled nursing facilities
Culture: Detail-oriented, outcome-focused, stable, people-oriented, and team-oriented
Responsibilities
- Partner with the facility's therapy department to provide the best possible rehab care
- Communicate and collaborate with the patient's primary care team to ensure the best outcomes
- Complete timely charting and documentation (can be done from home)
Requirements
- Licensed Nurse Practitioner (Required)
- Certified Nurse Practitioner (Required)
- Licensed Physician Assistant (Required)
- Certified Physician Assistant (Required)
- Must be licensed in MI
- Reliable transportation
