About the Role
Join our team of experts in rehabilitation at Total Extended Care Services, a growing Physical Medicine and Rehab group. In this role, you will partner with our PM&R physician in the sub-acute rehab setting, working within an interdisciplinary, specialized team to improve patient outcomes.
Location: In-person at skilled nursing facilities in Michigan.
Schedule: Flexible options available for both full-time and part-time positions.

Responsibilities
- Partner with the facility's therapy department to provide the best possible rehab care.
- Communicate and collaborate with the patient's primary care team to ensure optimal outcomes.
- Complete timely charting and documentation (can be done from home).
- Provide on-site pain management with a focus on reducing re-admission rates.
- Coordinate care with medical providers, therapists, social workers, nurses, and families.
Requirements
- Licensed Nurse Practitioner (Required).
- Certified Nurse Practitioner (Required).
- Licensed Physician Assistant (Required).
- Certified Physician Assistant (Required).
- Must be licensed in Michigan.
- Reliable transportation.
