Responsibilities

• Deliver comprehensive primary care services, including preventive screenings, diagnosis, and treatment.
• Integrate Social Determinants of Health (SDOH) screenings and interventions into patient assessments and care plans.
• Participate in continuous quality improvement initiatives to meet and exceed Patient-Centered Medical Home (PCMH) quality metrics.

Requirements

• Active Nurse Practitioner license and DEA certification for Louisiana.
• Experience in primary care, acute care, or urgent care preferred.
• Strong critical thinking, communication, and independent decision-making skills.
• Passion for flexible, patient-centered care in metro and underserved areas.
