Nurse Practitioner- Outpatient Primary Care SC - ISM, Mount Sinai Hospital - Full Time - Day

About the Role

A Nurse Practitioner (NP) at Mount Sinai Hospital is an independent practitioner who diagnoses, treats, and prescribes for patients' conditions within their specialty area of practice, as defined by New York Education Law. Medical orders, progress notes, and treatment plans do not require countersignature by a physician.

Responsibilities

Clinical Practice

• Assesses the health status of patients/families through comprehensive health history and physical examination.
• Initiates and orders therapeutic and diagnostic procedures, consultations, and laboratory examinations; prescribes medication regimens; interprets findings and alters the plan of care according to accepted standards.
• Educates and counsels patients in health promotion, maintenance, disease prevention, and management of acute/chronic illnesses.
• Documents findings and plan of care in the patient’s medical record.
• Consults with physicians according to established standards, policies, procedures, and protocols.
• Demonstrates knowledge and skills necessary to provide care appropriate to the age of patients served.
• Initiates referrals to specialty services and fosters continuity of care.
• Participates in team meetings and conferences to enhance an interdisciplinary approach to care delivery.
• Maintains patient and employee confidentiality.
• Develops and records the plan of care based on diagnoses and individual patient needs; interprets client responses to the plan of care.
• Performs patient record reviews with the collaborating physician at least every three months if utilizing a collaborative practice agreement.
• Completes the Focused Professional Practice Proctoring Evaluation (FPPE) or Ongoing Professional Practice Proctoring Evaluation (OPPE) as required for credentialing.
• Practices as a member of the allied health staff according to Medical Staff rules and hospital bylaws.

Requirements

• New York State License with current registration as a Registered Nurse.
• Certification by the New York State Department of Education as a Nurse Practitioner with current registration:
  • Graduate of a New York State registered Nurse Practitioner Program or equivalent, or
  • Certified as a Nurse Practitioner by a national association via examination.
• Completion of a pharmacy component (minimum required semester hours or equivalent) and a 3-hour course on New York State and Federal laws relating to prescriptions and recordkeeping.
• Collaborative Practice Agreement with a physician and designated protocols if less than 3,600 hours of NP experience, or Collaborative Relationship with a physician if employed at Mount Sinai Hospital with over 3,600 practice hours.
• Master’s degree in a program designed to prepare an NP to diagnose, treat, and prescribe, or successful completion of an approved post-master’s NP certificate program.
• Previous progressive experience relevant to the area of clinical practice.
• National Board Certification in Advanced Practice Nursing required (deferred up to 1 year if approved by Chief Nursing Officer).
• Credentialed through the Credentialing Committee of the Medical Board with approved Delineation of Privileges.
• Drug Enforcement Administration (DEA) certificate and Basic Life Support (BLS) certification required.
• Advanced Cardiac Life Support (ACLS), Pediatric Advanced Life Support (PALS), Neonatal Advanced Life Support (NALS), or Neonatal Resuscitation Program (NRP) certificates as required by specialty.
