Nurse Practitioner-Geriatric/Palliative SC - ISM, Mount Sinai Hospital - Full Time - Day

Full-time Nurse Practitioner role in Geriatric/Palliative care at Mount Sinai Hospital, specializing in independent clinical practice as defined by New York Education Law Title VIII, Article 139 section 6910. NPs diagnose, treat, and prescribe without physician countersignature.

Responsibilities

• Assess patient/family health status through comprehensive history and physical examination.
• Initiate and order therapeutic/diagnostic procedures, consultations, lab tests; prescribe medications; interpret findings; adjust care plans per accepted standards.
• Educate and counsel on health promotion, disease prevention, and management of acute/chronic illnesses.
• Document findings and care plans in medical records; consult with physicians per established protocols.
• Provide age-appropriate care based on physical, psychosocial, educational, and safety criteria.
• Initiate referrals to specialty services and ensure continuity of care.
• Participate in interdisciplinary team meetings and conferences.
• Maintain patient/employee confidentiality and manage information securely.
• Develop individualized care plans; record and interpret patient responses.
• Conduct patient record reviews with collaborating physician at least every three months if under a collaborative practice agreement.
• Complete Focused Professional Practice Proctoring Evaluation (FPPE) or Ongoing Professional Practice Proctoring Evaluation (OPPE) for credentialing.
• Practice as an allied health staff member per Medical Staff rules and hospital bylaws.
• Support the mission, vision, and goals of the Department of Nursing and Mount Sinai Health System (MSHS).
• Demonstrate accountability for nursing practice; adhere to ethical codes and humanistic care standards.
• Serve as a resource for nurses and healthcare team members; participate in peer review.
• Incorporate research findings into clinical practice; participate in approved nursing/medical research studies.
• Maintain current knowledge through self-directed learning, literature review, and educational programs.
• Contribute to defining and interpreting nursing practice standards; enhance professional nursing image.
• Participate in departmental/interdisciplinary committees influencing nursing practice and patient care policies.
• Seek opportunities to improve practice and outcomes; treat all individuals with dignity and respect.

Requirements

• Master’s degree in a Nurse Practitioner program or post-master’s NP certificate.
• New York State RN license and NP certification with current registration.
• Graduation from a NYS-registered NP program or equivalent, or national NP certification via examination.
• Completion of pharmacy coursework (minimum required semester hours) and a 3-hour course on NYS/Federal prescription laws.
• Collaborative Practice Agreement with a physician (if <3,600 practice hours) or Collaborative Relationship Attestation (if >3,600 hours at Mount Sinai).
• Progressive clinical experience relevant to geriatric/palliative care.
• National Board Certification in Advanced Practice Nursing (required for hires after June 1, 2019, or for NPs billing professional services; may be deferred 1 year with approval).
• Credentialing through the Medical Board’s Credentialing Committee with approved Delineation of Privileges.
• Active Drug Enforcement Administration (DEA) certificate and Basic Life Support (BLS) certification.
• Specialty-specific certifications (e.g., ACLS, PALS, NALS, NRP) as required.
