About the role
Our Nurse Practitioners serve local communities throughout South Carolina, providing direct clinical care to clients in residential homes, assisted living, and independent-living facility settings. We offer Primary and Palliative Care for home-bound individuals, those with transportation challenges, and the geriatric population.
Responsibilities
- Work with an interdisciplinary team.
- Provide medical care within scope of practice.
- Complete yearly physicals.
- Provide education and counseling on preventative care and medical management of chronic illnesses.
- Manage and treat disease processes, including ordering lab tests, radiology, and referrals.
- Review and manage test results.
- Develop treatment plans, prescribe medications, and make referrals.
- Communicate with patients, families, and facilities regularly.
- Coordinate and provide patient-centered care.
- Provide high-quality care by identifying and assessing health concerns.
- Facilitate transitional care to hospice services.
- Manage medication regimens, order and review lab work and radiology.
- Demonstrate proficiency in documentation within the EHR.
- Maintain continuing education and attend conferences.
Requirements
- Master’s degree in nursing or postmaster certification as an APRN.
- ANCC or AANP Certified.
- Current South Carolina licensure.
- CPR Certified.
- Current Driver’s License.
- DEA License.
- Minimum 2 years of experience preferred.
Flexible 40-week schedule, Monday–Friday, with local travel required.
