Position Overview
A mission-driven community healthcare organization in New York City is seeking an experienced Nurse Practitioner or Physician Assistant to join its clinical team. The Advanced Practice Provider will deliver comprehensive primary and preventive care to a diverse patient population, including pediatric, adolescent, and adult patients. This role offers the opportunity to provide hands-on patient care while collaborating closely with physicians, clinical leadership, medical assistants, and other members of the care team. Clinical Responsibilities
- Provide comprehensive medical care to pediatric, adolescent, and adult patients.
- Obtain and document complete and relevant patient histories.
- Assess patients, identify clinical needs, and establish appropriate diagnoses.
- Develop and implement individualized treatment plans utilizing clinical findings and diagnostic information.
- Perform routine wellness visits and comprehensive physical examinations.
- Conduct gynecological histories and examinations when clinically appropriate.
- Prescribe medications within scope of practice and educate patients regarding proper medication use.
- Complete required medical documentation, forms, and clinical records accurately and within established timelines.
- Coordinate referrals to specialists and outside healthcare providers and ensure appropriate follow-up.
- Assist with immunizations, phlebotomy, and other clinical functions when necessary to support efficient patient care.
- Collaborate with physicians and specialists regarding consultations, referrals, and complex patient needs.
- Participate in regular clinical collaboration and case discussions to promote evidence-based, high-quality care.
- Participate in peer review and other clinical quality initiatives.
- Promote preventive medicine, disease prevention, and long-term health maintenance.
- Provide patient education and counseling related to chronic disease management, nutrition, prenatal health, family planning, medication management, parenting, stress management, and other relevant health topics.
- Advocate for patients and assist them in navigating appropriate healthcare services and resources.
- Participate in patient outreach and engagement initiatives as needed.
- Provide occasional coverage at additional organizational locations throughout New York City.
Administrative & Team Responsibilities
- Maintain compliance with HIPAA requirements and organizational policies and procedures.
- Provide clinical guidance and oversight to medical assistants when appropriate.
- Support medical assistants with patient care, triage, and workflow to maintain efficient clinic operations.
- Communicate with clinical and operational leadership regarding patient flow and care-delivery needs.
- Participate in multidisciplinary case conferences and clinical meetings.
- Assist clinical leadership with the development and implementation of medical protocols, workflows, and clinical documentation.
- Participate in healthcare-related research or quality initiatives when appropriate.
- Identify and recommend opportunities to improve patient care, clinical workflows, and the overall patient experience.
- Work collaboratively with clinical and administrative teams to maintain a patient-centered care environment.
Ideal Candidate
