About the Role

The Nurse Practitioner or Physician Assistant in Westminster will deliver high-quality outpatient care to a diverse patient population. This role focuses on comprehensive patient assessment, diagnosis, and management of acute and chronic conditions. You will collaborate with physicians, nurses, and other healthcare professionals to develop and implement individualized care plans, educate patients and families on disease prevention and health maintenance, and enhance patient outcomes through compassionate, evidence-based care.

Responsibilities

• Conduct thorough patient histories and physical examinations to assess health status and identify medical conditions.
• Diagnose and manage acute and chronic illnesses, prescribing medications and therapies as appropriate within scope of practice.
• Develop and implement individualized treatment plans, including follow-up care and referrals to specialists when necessary.
• Provide patient education on disease prevention, medication adherence, and lifestyle modifications to support long-term health.
• Collaborate with interdisciplinary healthcare teams to ensure continuity of care and optimize patient outcomes.
• Maintain accurate and detailed medical records in compliance with regulatory standards and organizational policies.
• Participate in quality improvement initiatives and stay current with clinical guidelines and best practices.

Requirements

• Current and unrestricted Nurse Practitioner (NP) or Physician Assistant (PA) license in the state of practice.
• Current DEA Registration.
• For NP: Prescriptive Authority.
• Master’s degree from an accredited NP or PA program.
• Certification by a nationally recognized certifying body (e.g., AANP, ANCC, NCCPA).
• Demonstrated experience in outpatient care management, including patient assessment and treatment planning.
• Basic Life Support (BLS) certification (must maintain throughout employment).

Preferred Qualifications

• Experience working in a primary care or specialty outpatient clinic setting.
• Familiarity with electronic health record (EHR) systems and telehealth platforms.
• Advanced Cardiac Life Support (ACLS) certification.
• Additional training or certification in chronic disease management or preventive care.
• Strong communication skills with a focus on patient-centered care.

Skills

The role requires strong outpatient care management skills for evaluating and treating patients in a non-hospital setting. Daily use of clinical reasoning and diagnostic skills is essential to manage a wide range of health conditions while coordinating care with other providers. Proficiency with electronic health records and telehealth technologies enhances efficiency and patient access. Strong communication and patient education skills foster trust and adherence to treatment plans. Skills in quality improvement and evidence-based practice support continuous enhancement of care delivery.

Working Conditions

Pay

$110,000 - $140,000 yearly salary. The estimate represents the typical salary range for full-time employees based on skills, experience, and comparison to others in the role.
