Role Overview
The Advanced Practice Clinician (NP or PA) provides comprehensive care to high-risk health plan members within a Senior Community Care program.
Responsibilities
- Conducting assessments
- Medication reconciliation
- Supporting short-term rehabilitation
- Documenting conditions accurately
- Collaborating with the care team
- Engaging in advanced illness and care planning conversations
- Counseling patients and caregivers
- Ensuring seamless coordination of care across settings
- Maintaining compliance with licensing and regulatory requirements
Qualifications & Skills
- Active certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care (NP) or NCCPA certification (PA)
- Unrestricted state licensure and DEA prescriptive authority, or ability to obtain
- Reliable transportation and a valid driver’s license
- PREFERRED: 1+ years clinical experience, especially in geriatrics, long-term care, or home care, with proficiency in EMRs and technology
