About the role
This job is an integral part of the care delivery system working together to provide a healing environment, by placing our customers first. The incumbent provides primary healthcare services for non-acute or chronic health conditions in specific populations in accordance with applicable scope and standards of practice and with the policies, values, and mission of the organization within the CRNP scope of practice.
Responsibilities
- Provides appropriate patient care to include recording patient medical history and physical assessment, developing therapeutic care plan, ordering diagnostic tests, interpreting test results, providing accurate medical record documentation.
- Communicates and coordinates patient care with patient, family, community health services and interdisciplinary team. Involves attending physician regarding plan of care and involves physician in clinical decision making of any issues that are outside the CRNP's knowledge base or scope of practice.
- Provides monitoring and continuity of care and coordinates periodic evaluation by physicians as defined in the written agreement between the CRNP and the supervising physician(s).
- Prepares written prescription orders for drugs and controlled substances, administers medication and injections, sutures wounds and provides wound care.
- Educates patients and family members regarding health and preventive medicine, medications prescribed, treatment instructions and recommends community resources as appropriate.
- Triages patient telephone calls, provides monitoring and continuity of care.
- Participates, develops, and implements performance and/or quality improvement initiatives. Ensures compliance with all regulatory agencies.
- May perform other duties such as manage outpatient concerns and any other duty specific to specialty.
Qualifications
- Required: Master's degree MSN from the practicing State Board of Nursing Approved Master's Program
- National Board Certification for Nurse Practitioners through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), National Certification Corporation (NCC), or Pediatric Nursing Certification Board (PNCB)
- Practicing-state licensure as a Nurse Practitioner Prescriptive Authority
- Within 60 days of hire: Drug Enforcement Agency Licensure, CPR - American Heart Association, Act 33 Child Abuse Clearance Certificate, Act 34 Criminal Background Clearance Certificate, Act 73 FBI Fingerprinting Criminal Background Clearance Certificate
- Preferred: Four to ten years related experience and/or training; or equivalent combination of education and experience.
