About the role
This role offers the opportunity to earn an annual quality-based bonus tied to population health initiatives and value-based metrics. The incumbent provides primary healthcare services for non-acute or chronic health conditions.
Responsibilities
- Provides appropriate patient care including recording medical history and physical assessment, developing care plans, ordering tests, interpreting results, and documenting accurately.
- Communicates and coordinates care with patients, families, and interdisciplinary teams, involving the attending physician in clinical decisions.
- Maintains continuity of care and coordinates periodic evaluations by physicians.
- Prepares and administers prescriptions, injects medications, sutures wounds, and educates patients and families about health, medications, and treatment instructions.
- Triage patient calls, monitors continuity of care, and participates in performance and quality improvement initiatives.
- Performs other duties as assigned or requested.
Qualifications
- Master's degree in Nursing from a Board of Nursing-approved program.
- National Board Certification for Nurse Practitioners through AANP, ANCC, NCC, or PNCB.
- Practicing-state licensure as a Nurse Practitioner with Prescriptive Authority.
- Within 60 days of hire: DEA license, CPR certification, background clearances.
Preferred Qualifications
- Master’s degree in Physician Assistant Studies.
- PA-C or board eligible, with ongoing recertification.
- Within 90 days of hire: DEA license, CPR certification, background clearances.
Substitutions
Four to ten years of related experience and/or training, or equivalent combination of education and experience.
