Essential Duties & Responsibilities
- Deliver compassionate, expert care by completing patient histories, physical exams, and care plans tailored to each patient’s needs.
- Facilitate meaningful conversations with patients and families to establish goals of care and ensure choices are clearly documented and honored.
- Guide advance care planning, including completion of advance directives and POLST forms.
- Provide symptom relief for pain, fatigue, shortness of breath, anxiety, nausea, and other distressing symptoms using pharmacologic and non-pharmacologic approaches.
- Collaborate daily with the hospital’s interdisciplinary team to ensure seamless, patient-centered care.
- Educate patients, families, and hospital staff on palliative care principles, treatment options, and next steps in care.
- Exercise autonomy in decision-making, serving as a trusted resource for patients, families, and colleagues.
- Maintain accurate documentation in medical records while ensuring compassionate communication at every touchpoint.
Qualifications
- Valid nurse practitioner or physician assistant license in state of operations & current board certification in your field
- Masters Degree in Nursing or Physician Assistant Studies
- Open to new graduates with prior hospital RN experience strongly preferred, ideally in ER or ICU settings
- Strong leadership, interpersonal, and problem-solving skills preferred
- Must possess excellent written and oral communication skills
- Must possess traits of punctuality, organizational skills, and ability to function within a team
