About the role
Join an innovative healthcare team as a HouseCalls Advanced Practice Clinician (Nurse Practitioner or Physician Assistant).
Responsibilities
- Conduct comprehensive in-home health assessments, including reviewing medical history, medication reconciliation, vital signs, and physical exams.
- Identify diagnoses, document findings, and communicate with primary care providers to address care gaps.
- Recognize urgent/emergent situations and provide appropriate interventions.
- Educate members on disease management, medications, and adherence, while addressing social determinants of health and providing referrals.
- Collaborate within an interdisciplinary team to promote preventive care and improve health outcomes.
Requirements
- Active, unrestricted licensure in your practicing state and relevant certifications (ANCC, AANP, NCCPA).
- Reliable transportation, ability to navigate varied home environments, and physical ability to perform assessments.
Qualifications
- Preferred: 1+ years clinical experience, proficiency with electronic medical records, and strong communication skills.
