Nurse Practitioner or Physician Assistant, HouseCalls- Erie, Wyoming Counties

About the Role

As a HouseCalls Advanced Practice Clinician (Nurse Practitioner or Physician Assistant), you will conduct comprehensive in-home health assessments for Medicare Advantage members, focusing on preventive care, education, and care gap closure.

Responsibilities

• Perform in-home assessments, screenings, and point-of-care testing
• Identify and document health conditions for care management
• Collaborate with primary care providers (PCPs) to address care gaps
• Educate members on health management and medication adherence
• Recognize urgent or emergent situations and intervene appropriately
• Address social determinants of health and provide necessary referrals

Requirements

• Active licensure as a Nurse Practitioner (NP) or Physician Assistant (PA) in the practicing state
• Certification (ANCC, AANP, or NCCPA)
• Reliable transportation and ability to navigate home environments
• Proficiency with electronic medical records and technology

Qualifications

Preferred: 1+ years of clinical experience, especially in geriatrics or home health, and strong communication skills with Medicare populations.

Pay

Competitive salary range of $125,000 - $175,000.

Schedule

Flexible scheduling (8:00 AM – 8:00 PM, 7 days a week) with no on-call, weekends, or holidays required.
