Nurse Practitioner or Physician Assistant - Annual Wellness Visit Program -Phoenix Metro Area

Key Responsibilities

• Providing patient care within the full scope of practice
• Managing a low patient load of 10 per day for quality care
• Utilizing EPIC EMR system with AI-powered notetaking tools
• Serving patients at multiple locations in the Phoenix area
• Working weekdays with no call responsibilities

Qualifications

• AZ license or ability to obtain national certification (AANC, AANP, NCCPA)
• Current DEA and BLS certifications or ability to acquire
• Minimum of 3 years primary care experience

Preferred Skills

• Experience with Value Based Care
• Knowledge of HEDIS, CAHPS, HCC Coding
• Proficiency in Medicare Wellness visits and EMR systems

Compensation

Salary ranges from $109,500 to $164,000 annually, with comprehensive benefits including health coverage, CME allowance, PTO, retirement plans, and potential bonuses.
