About the role

Transform bedside care with AI. As Circadia’s on-site Nurse Practitioner, you’ll see patients first, act on predictive health signals in real time, and coach facility teams to turn data into earlier interventions—reducing preventable readmissions. This is a full-time, embedded role requiring Monday–Friday, on-site presence at our partner facility, Cedar Creek Post Acute in Mt. Juliet, TN.

Responsibilities

• Direct patient care: Conduct initial and follow-up assessments on flagged residents; initiate/adjust plans of care; complete consent; coordinate with facility providers.
• Tech → bedside integration: Train and coach staff at the point of care, standardize use of Circadia into clinical workflows.
• Data into action: Interpret trend-based insights, prioritize high-risk patients, and drive timely interventions to prevent decline.
• Close the loop: Complete documentation/charts within the same day.
• High-volume rounding: Efficiently complete visits across designated SNFs (typically 25+/day where appropriate), focusing on new admits, facility-flagged patients, and Circadia alerts.
• Quality & product feedback: Surface field insights to improve workflows and tech integration in a clinical setting.
• Mentor & collaborate: Support NP onboarding, share best practices, and partner with clinical ops/product teams.

How Success is Measured (KPIs)

• Visits: Target 25 patient visits/day.
• Engagement: ≥80% Time to Acknowledgement Rate.
• Documentation: Complete all notes using AI-powered tools within 24 hours of each encounter.
• Adoption & impact: Demonstrated workflow adherence at facilities; trend toward fewer falls/readmissions over time.

Requirements

• Certified and licensed Nurse Practitioner in TN; ANCC (or equivalent) in Adult/Family/Geriatric/Acute.
• Eligible for Medicare participation; company bills on your behalf.
• 2+ years NP experience with complex adult populations; able to practice independently.
• Valid driver’s license; comfortable traveling locally.
• Thrives in a dynamic, evolving startup environment.
• English required.

Nice-to-Haves

• Tech-savvy: Google Suite, multiple EMRs, Slack, AI transcription/tools.
• Spanish is preferred.

Pay

The anticipated annual base salary range for this full-time position is $120,000 - $140,000. The base range is determined by role and level, and placement within the range will depend on skills, qualifications, experience, and location.
