Purpose of Your Job Description

The purpose is to support the physician practices department and to provide coverage when the providers are out of the office.

Delegation of Authority

They will report to the providers they work with and will utilize our nurses and MA’s in the offices as well.

Job Functions

  • Diagnose and Treat acute and chronic Illness
  • Perform age-appropriate history and physical
  • Order and interpret diagnostic and therapeutic tests relative to patient’s age-specific needs
  • Prescribe appropriate pharmacologic and non-pharmacologic treatments to patient’s age specific need
  • Maintain and monitor the effectiveness of treatments
  • Provide education of acute and chronic medical conditions
  • Manage Chronic Medical Conditions
  • Reconcile medication list
  • Facilitate appointments to specialist
  • Provide thorough exams and evaluations of patient’s health status
  • Facilitate appropriate chronic care management (e.g., Diabetic eye exams, foot exams and nutritional counseling)
  • Preventive Medicine
  • Keep track of and refer patient to appropriate preventive medical screenings (e.g., Mammograms and Colonoscopies)
  • Counsel and reinforce patients in healthy lifestyle habits

Qualifications

  • Education Requirements: Primary Care NP certified in Massachusetts
  • Experience: 3 years experience in a Primary Care setting
  • Proficient Computer Skills (MS Word)
  • EMR experience required
  • Knowledge of clinical reminders and how to use them appropriately
  • Good communication skills, both spoken and written
  • Able to change with the ever evolving healthcare field
  • Master Degree, ACLS, and PALS preferred
