Responsibilities

• Conducting up to hour-long in-home health assessments
• Reviewing medical history, medications, and social factors
• Administering tests

Qualifications

• Master's degree or equivalent
• Active NP licensure
• Board certification in adult/geriatric primary care
• CPR/BLS
• 1+ year experience
• Able to travel extensively
• Valid driver’s license
• Strong communication skills
