Nurse Practitioner - Full Time

About the Role

Provides in-home health assessments for members, reviewing medical history, social factors, and risks to improve care quality and address care gaps. Conducts adult/geriatric assessments, orders tests, collaborates with healthcare teams, and offers patient education. Services are delivered at members’ residences, including homes and Skilled Nursing Facilities.

Responsibilities

• Perform comprehensive health assessments and document findings.
• Administer and order tests as needed.
• Coordinate with physicians, case managers, social workers, and caregivers.
• Support patient education and follow-up in collaboration with primary care providers.
• Travel up to 15% to various locations.

Requirements

• Master’s Degree or equivalent with NP licensure and prescriptive authority.
• Board certification in relevant adult or family care specialties.
• Current BLS, ACLS, or CPR certification.
• Strong communication and computer skills.
• Ability to travel as required.
• Valid driver’s license and insurance coverage.
