Responsibilities
- Provide compassionate and dignified care to patients in their homes or hospice settings.
- Collaborate with interdisciplinary teams to develop and implement comprehensive care plans.
- Manage daily patient care activities, including medication administration, monitoring vital signs, and providing emotional support.
- Document all patient interactions and care activities accurately and timely.
- Participate in ongoing professional development to enhance clinical skills and knowledge.
Requirements
- Current licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN).
- Experience in home health or hospice care is preferred but not required.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Valid driver’s license and reliable transportation.
Qualifications
- High school diploma or equivalent.
- Completion of an accredited nursing program.
- Passing scores on relevant licensing exams.
Skills
- Excellent communication and interpersonal skills.
- Strong organizational and time management abilities.
- Ability to work effectively in a fast-paced, dynamic environment.
- Proficiency in Microsoft Office Suite.
