Nurse Practitioner - Hospice - PRN

Responsibilities

• Provide compassionate and dignified care to patients in their homes or hospice settings.
• Collaborate with interdisciplinary teams to develop and implement comprehensive care plans.
• Manage daily patient care activities, including medication administration, monitoring vital signs, and providing emotional support.
• Document all patient interactions and care activities accurately and timely.
• Participate in ongoing professional development to enhance clinical skills and knowledge.

Requirements

• Current licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN).
• Experience in home health or hospice care is preferred but not required.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Valid driver’s license and reliable transportation.

Qualifications

• High school diploma or equivalent.
• Completion of an accredited nursing program.
• Passing scores on relevant licensing exams.
